The study protocol was approved by the Ethical Board of the Nagoya University School of Medicine.
Women were excluded because of their small number. A total of 2,816 men participated in an annual health checkup program conducted in autumn of the baseline year 1997 and gave written informed consent to answering a lifestyle questionnaire and providing the results of routine biochemical analyses of blood. These analyses were performed in a laboratory which has regularly undergone both the internal and external quality control. We defined eligible subjects as those who were aged 40 to 59 years, completed the lifestyle questionnaire sheet, were not taking medication for diabetes or dyslipidemia, and underwent measurements of all of fasting TC, HDL-C, and TG. These eligibility criteria were met by 1,939 men who were recruited for the current investigation. Serial data of serum lipid and lipoprotein concentrations were collected annually until the health checkup in the year 2000, resignation, retirement, or job transfer, whichever came first, for a total of 7,056 examinations.

Informed consent for the use of personal information for analysis was given by the population of this study. The study protocol was approved by the Ethics Committee of Nagoya University Graduate School of Medicine, Nagoya, Japan.
For the present study, we obtained the subjects' actual weights at ages 20, 25, 30 years, and 5 years prior (in 1992) from the respective health checkup records. A complete data set was not available for 113 men. Seven men had a medical history of either cancer (5) or myocardial infarction (MI, 2) and they were excluded from the analyses, leaving 644 men.

Permission with written informed consent to look into the personal records of health check-up was given by all subjects on enrolling into the study.
The participants in the 1997 survey consisted of 2816 Japanese male workers aged 34?69 y. The survey included a self-reported questionnaire, physical examination, blood pressure measurement, and collection of blood samples for laboratory analysis. Finally, self-reported past weight, actual measured past weight, and both current weight and height were obtained from 2453 men (mean age: 50.1 y; mean elapsed period from age 25 y: 25.1 y).

The study protocol was approved by the Ethics Committee of Nagoya University Graduate School of Medicine, Nagoya, Japan.
In studying the possible association between long-term weight fluctuation and fasting serum insulin levels, we excluded participants without complete weight records (n=326) or insulin concentration (n=7). Those with a history of medically treated diabetes (n=62) or cancer (n=26) were also excluded because of the possible weight change or fluctuation attributable to them. Finally, 1932 individuals were left for analysis.
