New producer form

This platform is used by the GTFS community to drive the adoption of the GTFS Fares v2 extension project. By adding your organization to this database, here are a few awesome stuff that will happen: - you'll receive a notification when the fields you are using change stages (from experimental to adopted 😃 or outdated 😢) - you'll see if someone flags an issue with your dataset you'll motivate the working group by showing that their work is used - you'll help the community have visibility on what parts of GTFS-Fares v2 should be adopted next - you'll be part of an awesome community of GTFS enthusiasts 🚌🚄🚊⛴️
