Waitlist for a Good Food Box Subsidy

The Good Food Box is a grocery bag of fresh produce and eggs. Boxes contain a balanced variety of fresh fruits, fresh vegetables, root vegetables, leafy greens and eggs. By working with and purchasing from almost 40 Comox Valley farms, LUSH Valley is able to source a wide variety of high-quality in-season produce for Good Food Boxes. Please note our subsidized and no-cost Good Food Box streams are currently at capacity. Please fill in the application form below to join the waitlist for a Good Food Box subsidy. We are seeking additional funding to allow us to provide this service to more households, and will contact those on the waitlist as soon as possible if new spaces become available. Thank you for your patience. If you have an urgent need for food, please consider accessing emergency services through other service providers. Once we have new subsidized registration spaces available, LUSH Valley's Good Food Box subsidies (Boxes starting at $10/order) will be available for households with financial barriers to meeting their food needs. There will also be a limited number of no-cost spaces in the program, where the $10/order cost would prevent a household from participating. Included in these risks are negligence on the part of Lush Valley, its staff, board, members, officers, officials and volunteers, other participants, and owners of the facilities where the activities occur (referred to in the rest of this agreement as “Lush Valley and others”). I freely accept and fully assume all such risks and the possibility of personal injury, death, property damage, expense and related loss, including the loss of income. RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT In consideration of Lush Valley Food Action Society accepting my application to participate as a volunteer delivery driver, I agree: - To waive any and all claims I may have in the future against Lush Valley and others. - To release Lush Valley and others, from any and all liability for any personal injury, death, property damage, expense and related loss, including loss of income that I or my next of kin may suffer as a result in my participation in this activity, due to any cause whatsoever, including negligence, breach of contract or breach of any statutory duty of care. - To hold harmless and indemnify Lush Valley and others from any and all liability for any damage to or loss of property of, or personal injury to, any third party, resulting from my participation in this activity. - That this agreement is binding on not only myself but my next of kin, heirs, executors, administrators and assigns.
