Each benefit program within the Worker Benefits Program works a little differently. Check out our info sheets for each program's payment information, or take a look at our full program rate sheet.

Workers and/or owners at all USFWC member organizations are eligible so long as they meet the individual plan requirements. Most plans require workers to work a minimum of 20 hours per week on average. Independent contractors are not eligible.


Associate members of the USFWC must pay minimum annual dues of $500.

Yes. Each company can configure their worker benefits differently if they want to. The only caveat being that if they choose to make accident and disability voluntary, getting access to one gives access to the other, at no additional cost to the CTD partner. The administrative fee is $75 annually, per organization for access to both, and the individuals can choose what they need.

Yes, though they are not required to do so.

All USFWC members are welcome to join the plan during the annual open enrollment period, from October - early December. In June 2023, members who joined the USFWC on or after October 1, 2022 can enroll.

Workers can enroll as they become eligible throughout the year.

Dental insurance, vision insurance and health insurance deductions can be taken pre-tax as allowed for your organization under Section 125.

Our dental insurance company is Delta Dental, and our vision insurance is administered by Ameritas, offering access to the option of EyeMed or the VSP network.


As of 2023, insurance cards are only available digitally. However, as soon as coverage begins, workers may access their benefits using the group plan number along with their SSN/Tax ID # - dependents may need to use the SSN/Tax Id # of the worker who they receive benefits through. Email benefits@usworker.coop if you do not have the group plan number handy.


All insurance cards reflect the name of the employee who is enrolled. In the case of a dependent, for example, the name on the insurance card will be the name of the employee, and that’s what they can use when at the doctor’s office.


The Delta dental plan completely covers twice annual cleanings and exams, annual x-rays, and for children, annual fluoride treatment. It also covers more complex treatments. See the dental benefits description for more information.

The EyeMed and VSP vision options both completely covers an annual in-network eye exam with a $10 copay, and covers a significant portion of the cost of prescription glasses and contact lenses.

Because our members are spread out across the United States, we want to give you as much freedom as possible when choosing a dental provider.


Out-of-network benefits are the same as in-network benefits, making choosing a new dental provider or staying with your existing dental provider much easier.

You can unenroll if you are leaving your workplace or if your hours drop below 20 hours per week. There are also some specific qualifying events that make you eligible to drop coverage. But you can not re-enroll once you have dropped the plan until the next open enrollment period.


You can email us at benefits@usworker.coop to let us know that you would like to drop coverage or if you have questions about your eligibility to drop coverage. 

Your vision insurance is provided by EyeMed OR VSP networks through the Ameritas company. If you do not know which network you are covered by, you can check with your employer, check your Ameritas card, or log into your Ameritas online account.


Insurance cards are mailed to individually to participants, based on the addresses provided upon enrollment. As soon as coverage begins, workers may access their benefits using the group plan number along with their SSN/Tax ID # - dependents may need to use the SSN/Tax Id # of the worker who they receive benefits through. To find an eye doctor covered by EyeMed:


1.Go to https://www.eyemedvisioncare.com/locator/ (or eyemed.com > find an eye doctor)
2.Add in the Zip code for your area and choose “Insight” for the network
3.You may also prioritize the list according to your needs, including hours and scheduling, services, products, brands, gender, languages spoken, specialty, and accepting new patients.
4.Click “Get Results” 

Dependent children are eligible until the age of 26. Partners/spouses can be covered - legal marriage is not required, and partners of all genders are welcome. However, you should be able to show proof of shared bills or residence upon request by insurance companies.


We don’t require workplaces to contribute to non-workers’ monthly premiums.

We invoice participating workplaces monthly via email. These digital invoices include a "pay now" button, allowing your workplace to transfer money to the USFWC Member Benefits Trust. Invoices are sent through Intuit’s QuickBooks platform, which allows us your workplace to easily pay your benefits bills online. As of April 2020, we are offering an autopay option through Quickbooks, to increase the efficiency of our payment program. Contact benefits@usworker.coop to learn more and sign up!

Outside of the Open Enrollment period, workers may join the plan mid-year if they were hired within the past three months. You may also qualify to join mid-year as an individual if you are losing dental and/or vision coverage from another source and your workplace is already enrolled in USFWC health benefits.


For members that have joined the USFWC since October 1, 2022, there is an opportunity to join the worker benefits program in June 2023.

Open Enrollment for all new and renewing workplaces will take place from October 1, 2022 - December 7, 2022. Enrolling in disability insurance during this open enrollment period ensures guaranteed coverage, meaning you cannot be turned away for preexisting conditions.


Outside of the Open Enrollment period, workers at enrolled workplaces may join the plan mid-year, but they will not be guaranteed coverage for all conditions.


For new members to the USFWC, you are able to enroll if you have joined the USFWC membership in the last 3 months.

Here is a description of the benefits (only available in English)

The funds go directly to the insured individual, to be used as the individual chooses.

The employee receives the benefit.

For accident and disability insurance, workers may enroll at any time but may not have coverage for preexisting conditions and other exclusions.

For voluntary benefits (accident and disability), workers or workplaces (depending on the arrangement) are billed by Diversified Human Solutions on a monthly basis.


Annually, the USFWC bills participating organizations $75.

Disability insurance covers accident, illness, and pregnancy for which you received medical care, treatment, or medication and cannot work per a doctor's determination. Some exclusions may apply, and exact details are determined by each policy.

Any condition you received care, treatment, medication (or a medical professional RECOMMENDED you receive such remedies) in the 12 months immediately prior to the effective date of coverage. Some exclusions may apply, and exact details are determined by each policy.

Yes, the guaranteed issue is available throughout the year for people becoming eligible for the first time

Benefits are unlikely to be available within 9 months of the beginning of the policy. This may vary on a case-by-case basis, such as in the scenario of an early delivery, but is not guaranteed.

Yes, you can unenroll from accident or disability insurance at any time. However, if you want to re-enroll in disability insurance, you will have to re-qualify and may have to pay a higher rate.
