ICA Perspective: This question is partially redundant with Exit Destination, a HUD UDE, and Reason for Leaving (not a required question but is in every Exit collection screen), however it is more in-depth (the HUD Exit Destination does not have a conditional follow-up question for clients who are indicated to have been asked to leave the program). These exit questions are known to be extremely difficult for programs that have lower services engagement and/or shorter lengths of stay (ie many/most emergency shelters) to complete. State funders may wish to clarify to programs how/if this data point is utilized. If it is not being used, then removal is certainly worth considering.

I wonder if this level of detail is necessary to be captured in HMIS, or if it is being utilized at all. The LTH reports over the last few years have been very glitchy - not even accurately telling me if fields like this one are up to date or missing information. For nearly all of these programs information like address and subsidy amount is reported on elsewhere. Our staff is often involved in subsidy administration, collecting income documentation, leases, contracts addresses etc., and in some cases directly administering subsidy payments. All of that information is being more accurately captured in accounting and in our files. I would think that most funders would be analyzing their own data about subsidy administration over data entered into HMIS. Lastly, the glitchy reports also add an unnecessary administrative burden, The last few years of LTH I have spent many hours chasing down errors on my reports, only to discover that the information was entered correctly.

  • Adding more detail to the Data Collection Instructions for this question is a possible solution. The person who submitted this proposed edit referred to the Adult Shelter Connect (ASC), which is collecting data on behalf of single adults shelters in Hennepin County prior to the client's anticipated stay at a shelter. So at the time this data is collected, the client may not be staying in Hennepin, but if they do, the answer to "County where resides" will need to be Hennepin, which might not be the case when the ASC is talking to the person. ICA would recommend that the ASC enters the answer Hennepin for all clients they collect data for.

This questions should be removed from the Adult Shelter Connect intake process. Reasons: Most times the client will be forthcoming and tell us they are currently fleeing their abuser and that is why they are seeking shelter. It doesn’t need to be in the intake, it can just go in our stat sheet notes. We may trigger or re-traumatize the client during the intake process which can be very harmful especially if the intake is being conducted over the phone and there is no way to assess emotional needs and safety of the client. Men are often offended by this question. It will take some rapport building to begin to ask these types of questions to males an/or same-sex partners in particular. Also, it may be hard for same-sex and LGBTQ+ partners to disclose this information since we have limited shelter options that are designed for them in mind. The Domestic Violence shelters in MN are at capacity. It is not uncommon for a DV advocate to call the Adult Shelter Connect requesting shelte

-This question should be asked once clients are referred to a housing program not at the first point of entry such as the Adult Shelter Connect. We do intakes via phone and in-person. It is hard to get this information from someone over the phone or in a short meeting timespan that may only occur once. To get accurate information for this data element, the staff member should spend an extended period of time with the client and help them to map out their housing history to determine if they meet the current long-term homeless definition. It is hard for the client to determine if they have been homeless for 1 year or 4 times in the past 3 years versus just being homeless multiple times and not meeting the long-term homeless definition, especially during phone intakes/ short intake screenings.

This is a question that many of our front line staff experience as retraumatizing to participants at intake. Sometimes participants of our programs have in fact been in the foster care system, and in many cases those experiences were very traumatic. Entering into the social services system again through programs like SHS can sometimes remind people of that institutionalization that they have already experienced. It also can surface memories that are difficult for someone to share in that initial meeting, and that a family advocate who is not a trained therapist will not be equipped to meaningfully support the first time they connect with that family or individual.

Would be great if a more in depth explanation of the options was available. Often staff get confused between Medicaid, Medicare and State Insurance for Adults. I work at an org that marks everyone who has MA as Medicaid - which is clearly different than all the entries that come in who already have State insurance checked in HMIS. Staff don't know the difference between all three of these options.

Participants and staff cannot often discern between MA and MN Care. Perhaps the question could ask whether someone has a Minnesota-funded source of health insurance vs. employer provided, VA, Medicare, COBRA, etc. I know that Medicaid is federally-funded. But since MN also funds it and we call it Medical Assistance in this state, it is a MN heath care option.

This is a difficult outcome to get at. Once someone has exited our program, we have no guarantee of being able to be in contact with them again, and we really should not be working with participants who are no longer enrolled with us. This question puts us in the position of having to still be in contact with exited participants. In some cases those participants may in fact be experiencing homelessness again at the time we report this outcome. Calling a former participant to ask about their housing status when you are not able to offer them services in the event of homelessness is potentially harmful to the former program participant. I would recommend that for programs that utilize "Maintained Stable Housing After Exit" goals and outcomes for their program evaluation, that they move to seeing if the participant has been enrolled into a different HMIS program since exit.

This is a difficult outcome to get at. Once someone has exited our program, we have no guarantee of being able to be in contact with them again, and we really should not be working with participants who are no longer enrolled with us. This question puts us in the position of having to still be in contact with exited participants. In some cases those participants may in fact be experiencing homelessness again at the time we report this outcome. Calling a former participant to ask about their housing status when you are not able to offer them services in the event of homelessness is potentially harmful to the former program participant. I would recommend that for programs that utilize "Maintained Stable Housing After Exit" goals and outcomes for their program evaluation, that they move to seeing if the participant has been enrolled into a different HMIS program since exit.

Take away the day a benefit start and only require a the month and the year. A lot of Youth don't know/remember the start dates of things. Perhaps have an approx option without it being an error.

  • Because this is a HUD HMIS data element, we are unable to change the collection requirements. That said, the Non-Cash Benefits data element does not require a start date based on when the benefit began. As long as the sub-assessment's start date represents the date a benefit is current (i.e., the client is currently receiving that benefit), that is accurate data entry. ICA will follow up with the person who submitted this request.

Data Collection Instructions A permanent address is an apartment, house, or room where the client last lived for 90 days or more. Shelters and time-limited housing are not permanent addresses. -I think this question is somewhat confusing and may not be capturing the data we are hoping for. Just because someone is living in someone’s house for 90 days does not mean it was their permanent address, the client could have simply been couch hopping 3 months at a time or doing a short-term hotel/airbnb stay. Open to having more conversations.

I do not think that this is something that needs to be ask at during intake at Adult Shelter Connect. I think that it should be ask, if the person is going to be entering into a housing program. The answer to this question will not stop someone from getting into shelter.

Our committee is ambivalent about this question and would like to have clear rationale. Is this data point used across systems? This data point could be used to collaborate across these institutions for solution-oriented approaches to stop clogging the system with inappropriate discharges from programs/institutions. Are we trying to identify the areas that need our attention to close loopholes and gaps in the system? Programmatic feedback: this question is not necessary for shelter intake. It could be helpful for service coordination, but it is unlikely that case managers would look this up in HMIS as opposed to other means. We considered full removal of the question for shelters, however, we also discussed that if we only look this data point for those being served by a housing project, we would not be able to observe if less subpopulations are entering housing than enter shelter. While that is a reason why to ask it as a UDE, is it used?

Tracking the service dollars in HMIS in addition to the other ways we are required to track and report on it is an unnecessarily high administrative burden. I do SHS data entry for around 25 projects in Service point, only one of the 25 is an FHPAP program serving only 27 families at the moment, yet, this is the HMIS program that by far takes up the most of my time in HMIS - at times as much as 10 hours a month, 3-4 on the low end monthly. This is caused by the monthly reporting, and because we track our service dollars both here, and in our accounting system. I think that we should move to only tracking them in accounting, and moving away from monthly reporting for this program to lower the administrative burden on organizations delivering services.

The matrix options and range do not seem to carry equivalent rate within the report. For example, a 5 for Substance Abuse (6 months sober/abstinent) does not seem indicative of the same level of stability as a 5 on Disabilities or on Mental Health, at least not for someone with a history of Substance Abuse. Some of the questions seem to lack validity within their own range. For example on Health Care Coverage, a 3 rating indicates member(s) on a Public Health Insurance Plan, and a 5 indicates Affordable, Adequate Health Insurance. In MN, however, Medical Assistance is good, affordable and generally very adequate coverage. Additionally, related to Health Insurance, it seems at least as pertinent to ask whether an individual is actually attending/following through with doctor's visits and recommendations and managing their health care needs appropriately. The Self-Sufficency Matrix adds a high administrative burden to both administrative and direct service staff. It is not a helpful case management tool, and it actually can feel dehumanizing to the program participant. The definitions are often not a clear fit, which results in inconsistent data. Inconsistent data means that the data will not be effective in telling us as service providers or as a wider collaborative about the impact of our work on the communities we are trying to serve. This data has not historically been used or useful, I believe that for all these reasons, we should do away with it, rather than invest in better utilization of the data.

- hear from case managers all the time. Not helpful, demeaning. Big reporting burden. Leads to inconsistent data because it's so subjective- makes it not useful. Never received feedback about reports submitted. Knowing how harmful it is, is there room for us to get rid of it? - both as an administrator, case manager- innate ableism and racism in structure. Majority of case managers college educated on whether or not someone is…. No way to remove implicit bias from different case managers do these reports. To assume that every case manager is providing correct and reflective and helpful feedback on their participants is not possible. Have has seen this take place. Case manager has one idea of what "success" looks like and it is white middle class homogentisic. Ideas in the matrix are so harmful and I wouldn't want a participant to see it- so why are we collecting it? Even if you do it with your client, it is so dehumanizing. We would like to see the veteran series questions reduced. The only questions could be: 1. Did you serve in the U.S. Military. If yes, what kind of discharge did you have? If yes, the client should be referred to the Homeless Veteran Registry and other veteran resources.
Because some of the questions are insensitive to ask during an intake and can be triggering/re-traumatizing, especially if they had a negative experience in the military. Questions such as: For approximately how long did you serve (does it matter, a veteran is a veteran), What type of discharge did you have?, Do you receive VA Disability benefits?, etc. For example, the Adult Shelter Connect reaches out to veterans services when just the first question is true (are they a veteran) after intake for potential service delivery. The rest we do not need. Another perspective: many veterans do not want to engage with VA, so the asking the whole series of questions, even if they do not want to engage VA, can feel non-consensual. If kept, is there a way to only ask if the client chooses rather than required? These questions can remind clients of difficult interactions with the VA, as well.

I think it may be a good idea to remove this question and only evaluate clients on their actual rental history. If they did not get a formal eviction from the program then we do not need to know the reason they were asked to leave the program.
