Kollective8 Arts Studio | Freeze Membership Form

For month-to-month memberships, you may freeze your membership with AT LEAST (2) week notice prior to your scheduled reoccurring payment for a low ONE-TIME fee of $25 to maintain your current grandfathered membership rate. Example: If your monthly payment is scheduled for the 25th of every month, you must submit a freeze request by the 9th. If you submit AFTER, or outside the two-week window date, freeze will take effect the following month.
