5-10 minute Recommendation Form for faculty endorsing students to the Cornell Prison Education Program

Cornell undergraduates volunteer for the Cornell Prison Education program in a number of capacities (tutoring in prison, helping run our in-prison computer labs, etc.) When undergraduates apply, we ask that all such applicants recruit a recommendation from one of their professors or other mentors. We have created the below form in order expedite the process.

Name the person for whom you are submitting this recommendation.
E.g., "The student took a class with me." "The student is an undergraduate assistant in my lab." Etc.
We use the Net ID or email of the person as a record locator in our application processes.
