We can only facilitate vaccine appointments for clients and current patients that are up to date with their wellness exams. New clients or current clients with new patients should call to schedule a wellness visit prior to making an appointment for vaccination. By filling out this form, you are scheduling both the initial vaccine and a booster in 21 days. Fill out the form separately for each rabbit. A confirmation text message will be sent two days prior to the appointment. A non-refundable payment for initial vaccination and booster will be due at the time of your appointment.
