Part Files *
Attach the part file of the part you want printed. We accept STL, OBJ, or 3MF format.
If you attach your own .form file, please note that we will print the file as submitted and you will be charged for any failed prints as a result of the tray setup.
Please do NOT attach a .form file unless you are familiar with and have optimized your print results on an SLA printer. You may enter specific orientation instructions in this order form if needed and we will reach out if there are issues with your requested part orientation.

The Color/Material of My Part is NOT Important
Please check this box if color/material is not important to your part. Your print may be processed quicker if you select this option because we can process multiple orders that share materials on the Form 2, so long as they fit on the print bed.
Optional - If necessary, please specify part orientation.
**Important** The Form 2 prints parts best at an angle, which is very different than how one would orient a FDM part. We recommend that you ask for a specific Part Orientation only if you are familiar with and have optimized your print results on an SLA printer.

Skip Post - Cure
Some materials require a post cure. Other materials recommend it. We will cure your Form 2 print with the supports attached unless you request us not to.
FormLabs recommends curing with supports attached, but some people prefer to remove their supports prior. If you would like to pick up your parts to remove supports, skipping the UV cure step, please check this box.

Payment Type *
If you select "Credit Card / Debit Card" we will send you a link by email in order to collect payment. We have a $1 minimum for this payment type, so the cost will be rounded up to reach this minimum.
Sears think[box] Accounts are prepaid accounts typically associated with classes, events, or programs. Sears think[box] Accounts are a seven-digit number (i.e. 1234567)
CWRU Speedtypes are institutional accounts managed by the university. Speedtypes consist of a three-letter prefix followed by a six-digit number (i.e. ABC123456)
CaseCash, cash, and cheque payments will be processed in-person. If using one of these methods, payment will not be required until the time of pickup.
