Not every person who desires to adopt a dog will be approved for adoption. A Home 4 Ever retains the discretion to determine the best fit for the dog. A Home 4 Ever does not sell dogs but instead adopts them to a home where it is expected they will be loved and cared for as another family member. Every dog adopted through A Home 4 Ever Rescue has been spayed or neutered prior to adoption. By completing this questionnaire, you will aid us in determining if you and your family are indeed ready for pet ownership, and if the dog of your interest would suit you and your lifestyle. Should you agree that adopting a pet is a commitment throughout the lifetime of your companion animal, please fill out this questionnaire. Please note that A Home 4 Ever will verify the information you provide and any misrepresentation will automatically disqualify a person from receiving a dog. Please understand that A Home 4 Ever is a non-profit organization run entirely by volunteers with other jobs and obligations.
