SAR to the Department

The purpose of this form is to collect anonymous feedback from you on Subject Access Requests sent to the Department of Children requesting personal data from the archive of the Mother and Baby Homes Commission. This will help us to make our Guide and templates more useful. You don't have to have used our template to make your SAR in order to fill this out - the more feedback we get the better!
