Waiver of Liability, Assumption of Risk and Indemnity Agreement I affirm that I am voluntarily participating in club activities and further affirm that I acknowledge the inherent risks in all health and fitness club activities, that I am aware of and appreciate these risks and I assume all responsibility for personal injury, death, or loss resulting from these inherent risks. In consideration of permission to use, today and on all future dates, the property, facilities, and services of the Chamisal Tennis and Fitness Club, I do hereby release Chamisal, its owners, employees and agents from any and all liability including negligence of Chamisal et al. I, for myself, my family, heirs, personal representatives or assigns, do hereby, hold harmless, release and covenant not to sue Chamisal Tennis and Fitness Club, its parent company, directors, offices, employees and agents, for any claims including negligence which may result in personal injury, accident or illness (including death), and property loss arising from, but not limited to, participation in club activities, observation, and use of facilities, premises, or equipment. I affirm that I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
